Chocolate can be part of a healthy diet when people choose varieties with a high proportion of cacao, less added sugar, and eat it in moderation, according to a review of the scientific evidence published to mark International Chocolate Day, observed every September 13.



The reason lies in cacao's flavanols and other polyphenols, compounds with biological activity that, according to available research, can favorably affect blood vessel function and improve certain markers linked to oxidative stress. But the choice should not rest on cacao percentage alone. The amount consumed and the overall diet also matter.

Dr. Katherine Reyes, a diabetes specialist and nutritionist consulted by Consulta Libre, said everything depends on the type of chocolate, its composition and the amount eaten. She noted that cacao contains bioactive compounds, particularly polyphenols and flavanols such as epicatechin and procyanidins, which have been widely studied for their possible effects on blood vessel function, oxidative stress and cardiovascular health.

Reyes cautioned that this does not make every chocolate product automatically healthy. Many commercial chocolates contain significant amounts of sugar, fat and calories, so cacao's potentially beneficial compounds do not turn chocolate into a food that can be eaten without limit. She said the evidence points to benefits linked mainly to products rich in cacao and flavanols, and that these should be understood within a balanced diet, appropriate portions and a healthy lifestyle overall.

Myths and realities about chocolate consumption



One common myth is that all chocolate is equally healthy. In reality, composition varies widely. Dark chocolate generally contains a higher proportion of cacao solids than milk chocolate, while white chocolate contains no cacao solids at all, consisting mainly of cacao butter, sugar and other ingredients. Chocolates with a higher cacao content tend to carry more of cacao's naturally occurring bioactive compounds, though the percentage of cacao does not necessarily reflect the exact flavanol content, since origin, fermentation, roasting and industrial processing can all alter polyphenol levels.

Another myth holds that eating more chocolate brings greater benefits. According to the evidence, chocolate, even dark chocolate, is energy-dense, and excessive consumption can significantly raise calorie intake and contribute to weight gain if it is not properly worked into the overall diet. Many of the studies on flavanols, moreover, use cacao products or extracts with standardized amounts of these compounds, not chocolate bars as sold in stores.

A third myth is that chocolate protects the heart and prevents heart attacks. Cacao can produce favorable effects on some mechanisms related to vascular health, but eating chocolate cannot be said to prevent heart attacks or strokes on its own. Cacao flavanols may promote production of nitric oxide, a molecule involved in relaxing and dilating blood vessels. A Cochrane review that assessed 35 studies found that consuming cacao products could produce a small short-term reduction in blood pressure, but the available studies did not directly show that eating chocolate reduces the incidence of heart attacks or strokes.

What type of chocolate offers the most benefits?



From a nutritional standpoint, the most interesting option tends to be dark chocolate with a high proportion of cacao and less added sugar. As a practical guide, chocolates with around 70% cacao or more are often recommended.

Cacao is an important source of polyphenols, especially flavanols such as epicatechin, catechin and procyanidins, which have antioxidant properties and may help modulate oxidative stress, an imbalance between oxidizing molecules and the body's defense systems. The most recent clinical evidence supports the idea that cacao and dark chocolate can favorably influence some biomarkers of this process.

How much chocolate is recommended?



There is no universal amount of chocolate that is necessary for everyone. As a practical guide, a moderate portion of roughly 10 to 20 grams of dark chocolate can be reasonable for many people within a balanced diet, equivalent to one or a few small squares depending on the product's size.

The Cochrane review on cacao products and blood pressure notes that flavanols have drawn interest for their potential to promote vasodilation through mechanisms related to nitric oxide. The available evidence points to small favorable effects on blood pressure, though long-term studies demonstrating a reduction in cardiovascular events such as heart attacks or strokes are still lacking.

What does science say about cacao and brain function?

The relationship between cacao flavanols and the brain remains an active area of research. It has been suggested that flavanols could influence mechanisms related to cerebral blood flow, inflammation and oxidative stress, but that does not mean eating chocolate automatically improves memory.

Large clinical studies have been more cautious. In a clinical sub-cohort of the COSMOS study, 573 older adults were evaluated over two years, and daily supplementation with a flavanol-rich cacao extract produced no significant benefit for global cognition, episodic memory or executive function in the general population studied. Another large trial within the COSMOS program likewise found no significant benefit of cacao extract on global cognition after three years of follow-up.

Why is chocolate linked to well-being?



The association between chocolate and well-being has several explanations and cannot be attributed to a single compound. Cacao contains theobromine and small amounts of caffeine, substances belonging to the methylxanthine group, which can produce stimulant effects, though theobromine's characteristics and effects differ from those of caffeine.

Chocolate also carries a strong sensory component. Its aroma, flavor, texture and the way it melts in the mouth all contribute to the experience of pleasure. There is an emotional and cultural component as well, since many people associate chocolate with celebrations, memories, gratification or moments of comfort, associations that can shape the subjective feeling of well-being.
