Older adults in the United States who receive a shingles vaccine face a significantly lower risk of developing dementia, according to a major study published in the journal Annals of Internal Medicine.

Researchers analyzed health data from 509,926 Americans aged 66 and older who were receiving treatment or long-term care in qualified nursing care facilities across the country. None of the participants had a diagnosed case of dementia when the investigation began, and individuals were tracked over a follow-up period of up to four years.

Reduced dementia risk among vaccinated seniors

The study evaluated medical outcomes among residents who received at least one dose of the recombinant shingles vaccine compared to those who remained unvaccinated against the virus. Recombinant vaccines use synthetically produced viral proteins to trigger a targeted immune response without using a live pathogen.

Over the four-year follow-up period, the risk of developing dementia stood at 18.8 percent among participants who received the vaccine, compared to 24.6 percent among those who had not been vaccinated. Researchers reported that this corresponds to a 24 percent relative reduction in dementia risk, with an absolute difference of 5.8 percentage points between the two groups.

Qualified nursing care facilities provide continuous medical supervision and personal assistance for older individuals with complex health needs. Because residents in these long-term care settings often face elevated health risks, understanding preventive measures is a key focus for geriatric medicine.

Study limitations and brain health findings

The study authors noted that their results add to previous evidence pointing toward a potential connection between shingles immunization and broader brain health in older populations. Shingles, also known as herpes zoster, is a painful condition caused by the reactivation of the varicella-zoster virus, the same pathogen responsible for chickenpox. The virus can lie dormant in nerve tissue for decades before re-emerging when immunity wanes in later life.

However, the researchers cautioned that further analytical tests revealed signs of residual influence from other unmeasured health or lifestyle factors. Consequently, the authors emphasized that their work does not prove that the shingles vaccine itself directly prevents dementia. Dementia is an overarching term for conditions characterized by severe decline in memory, reasoning, and daily cognitive functions.

The publication of the findings in the Annals of Internal Medicine, a flagship peer-reviewed medical journal established by the American College of Physicians, contributes to growing scientific inquiry into preventive treatments for cognitive decline. The research follows earlier reports indicating that hormone replacement therapy is associated with a reduced risk of dementia and Alzheimer's disease in women.
