Pilots and flight attendants have been identified as having the highest proportion of cancer deaths linked to ionizing radiation exposure among more than 500 occupations analyzed in a major medical study.
The findings, published in the peer-reviewed journal JAMA Internal Medicine, revealed that radiation-associated cancers accounted for 6.9 percent of all deaths among flight attendants and 6.7 percent of deaths among pilots.

Researchers reached their conclusions after analyzing mortality data from nearly 13 million deceased individuals representing 503 separate professions. The sample included approximately 14,000 pilots and 7,000 flight attendants.
To isolate workplace radiation risks from external influences, the study authors adjusted their calculations for several demographic and personal factors. These controlled variables included age, sex, education level, marital status, and other potential confounding health elements.

Elevated Rates of Specific Cancers
The investigation examined a specific group of malignancies classified as radiation-associated types of cancer. This category includes breast cancer, central nervous system tumors, multiple myeloma, leukemia, lymphomas, thyroid cancer, prostate cancer, melanoma, and other skin cancers.
When analyzing specific disease outcomes, researchers recorded statistically significant higher rates of death among both pilots and cabin crew for four major conditions. Both professions experienced elevated mortality from breast cancer, central nervous system tumors, prostate cancer, and melanoma.
Pilots were also found to face a statistically significant higher risk of death from leukemia compared to other occupational groups in the study.
Ionizing radiation consists of high-energy subatomic particles or electromagnetic waves that can penetrate biological tissues and alter cellular DNA. Over extended periods, such genetic damage can trigger cell mutations that develop into malignant tumors or blood disorders.
Impact of High-Altitude Cosmic Radiation
The main cause of the elevated cancer mortality rates among flight personnel is regular exposure to cosmic radiation during high-altitude flights, according to the study authors.
Cosmic radiation originates from solar activity and deep space phenomena outside the solar system. At ground level, Earth's dense atmospheric envelope shields humans from the vast majority of this cosmic background radiation.
Commercial airliners routinely cruise at altitudes between 30,000 and 40,000 feet, where the surrounding atmosphere is significantly thinner. At these high altitudes, cosmic radiation levels are substantially higher than at sea level, exposing aircrew to steady background radiation over their careers.
Because commercial flight crews log hundreds of flight hours each year, their cumulative annual radiation exposure can exceed that of many ground-based industrial and medical workers.
Calls for Stricter Exposure Limits
Based on the documented mortality rates, the study authors called for enhanced workplace safeguards to protect commercial aviation workers from cumulative radiation hazards.
The researchers stated that flight crews require stricter monitoring of accumulated radiation doses throughout their careers. They also recommended implementing additional protective measures for airline personnel to mitigate long-term exposure risks.
