The former president Jair Bolsonaro (PL) presented a clinical worsening, with elevation of blood pressure and worsening hepatic laboratory tests, according to a medical report released by DF Star Hospital on the morning of Thursday (24).
Bolsonaro would be subjected to new image tests later this Thursday. It remains hospitalized in the Intensive Care Unit (ICU) and has no forecast of discharge.
The former president underwent an exploratory laparotomy on the 13th for the release of intestinal adhesions and reconstruction of the abdominal wall.
According to Rodrigo Perez, head of the Coloproctology and Intestine Center of the Digestive Center of the German Hospital Oswaldo Cruz, similar to that of Bolsonaro may be related to common processes in the postoperative phase.
“When the patient has an intestinal obstruction and needs to be operated because of this, it is common for the intestine to be paralyzed in the postoperative period. It is what we call paralytic ileum. It is like an intestine response, a defense mechanism,” Perez notes to CNN .
Gut stop leads to the need for parenteral nutrition, which is intravenous feeding, and nasogastric probe, as the patient cannot feed. With this, it is common for him to present changes in laboratory tests, including the levels of liver (liver) enzymes, according to Perez.
“Parental nutrition, which is done directly in the patient’s vein, turns out to be slightly different from that food we eat directly and is absorbed by the gut. As well as the formulations, it is not exactly the same as we eat,” the expert adds. “It often results, as a result, in the elevation of liver enzymes,” he adds.
Regarding the elevation of blood pressure, Perez explains that a number of factors may be associated, such as a possible pain or discomfort sense by the patient. “Increased pressure requires care (…), but it doesn’t seem to me very worrying,” he says.
Role of stress in the clinical picture
The expert also adds that emotional factors such as stress and psychological pressure can contribute or impair patient recovery. However, the effect is “smaller than they think,” according to Perez.
In addition to arterial elevation and changes in liver exams, other complications may occur in similar clinical paintings. This is the case of bacterial translocation, a condition characterized by the circulation of bacteria located in the intestine to the bloodstream, causing an infectious picture.
“This is a picture that is generally easily circumventable, either from the functioning of the intestine itself, or with drainage of the gastric probe. But sometimes, it is necessary to use antibiotics,” concludes Perez.
